Setting Up an Intake Form

Important

As we integrate the OneTrust and Convercent Ethics and Compliance product lines, we will temporarily have information that lives in the myOneTrust and Converge communities and knowledge bases. This article is relevant for Helpline & Case Manager. You can configure the intake form field options that will be presented to anonymous users when they access your organization's Ethics Portal. This allows you to create multiple intake forms and assign them as the preferred or default intake option for anonymous submissions.

To add a custom field

  1. On the Whistleblower and Ethics menu, select Setup > Intake Forms. The Intake Forms screen appears.

  2. Select an Intake Form. The Intake Form Details screen appears.

  3. Click the Add New button. The Add Form Fields modal appears.

  4. Complete the fields, then click the Save button. For more information, see Add Form Field Screen References.

  5. The new question will appear in the Intake Form Details preview.

Field

Description

Name

Enter a name that best describes the question you are asking. The field name will appear in the question's empty field on the intake form.

Response Type

Select from the available response types that anonymous user will select from:

  • Text

  • Single Select

    Note

    You will need to add New Option Values from which the anonymous user will select. Click the Add button to add as many new selection options as needed.

  • Radio Button

  • Text Area

Required

Enable this setting to mark the question as required.

To edit form field settings

  1. On the Whistleblower and Ethics menu, select Setup > Intake Forms. The Intake Forms screen appears.

  2. Select an Intake Form. The Intake Form Details screen appears.

  3. Click the Edit icon next to a question.

  4. In the Settings tab, edit the Form Field Label, Response Type, and Required status.

  5. Click Save and Close to save your changes.

To edit form field options

  1. On the Whistleblower and Ethics menu, select Setup > Intake Forms. The Intake Forms screen appears.

  2. Select an Intake Form. The Intake Form Details screen appears.

  3. Click the Edit icon next to a question.

  4. In the Options tab, use the Add and Remove icons to configure the option values for the form field.

  5. Click Save and Close to save your changes.

To reorder questions

  1. On the Whistleblower and Ethics menu, select Setup > Intake Forms. The Intake Forms screen appears.

  2. Select an Intake Form. The Intake Form Details screen appears.

  3. Click the drag-and-drop icon to drag the question.

  4. Release to drop the question.

  5. Click the Save button.

To delete a question

  1. On the Whistleblower and Ethics menu, select Setup > Intake Forms. The Intake Forms screen appears.

  2. Select an Intake Form. The Intake Form Details screen appears.

  3. Click the Delete icon next to a question. The question will be deleted.

  4. Click the Save button.
